A permit written after the job is worse than no permit at all

Job safety analyses, toolbox talks, hot work and confined space permits, incident reports. The paperwork only makes anyone safer if it happens before the work — and it only survives an audit if it can be produced years later.

When it goes wrong

How a small gap turns into an expensive one

None of this is one bad day. Each one causes the next.

1

A JSA signed in the truck at the end of the week is not a hazard assessment, it is a signature. Everyone involved knows it, and it is the first thing an investigator looks for.

2

Crew sign-offs are the part that gets skipped, because collecting eight signatures on a paper form means eight people standing still at the one moment everybody wants to start.

3

When something does happen, the incident report is written under pressure, days later, by someone reconstructing a timeline from memory and text messages.

And then

An auditor finds a permit dated after the work it authorized, and every other record you hold becomes suspect at the same moment.

The same job, the other way

Where that chain gets broken

The same three steps every trade runs. The difference is that each one leaves a record the next one can use.

1

The JSA is completed on site before the job starts, with the hazards for that task already listed and a timestamp nobody can move.

2

Every crew member signs on the phone in turn — seconds each, on the spot — and the sign-off sheet is part of the same record, not a separate page that gets separated.

3

An incident report captures the scene while it is still the scene: photos, witnesses, conditions, all stamped with when and where, and routed to safety the moment it is submitted.

And then

Nothing gets re-typed, and the record exists before anyone asks for it.

Half an hour with your own data settles it faster than this page can. Start free →

The forms

8 ready-made safety and JSAs forms

Used across 3 trades — the same workflow, with the fields each trade actually needs.

Job Safety Analysis (JSA)

Pre-task hazard assessment with crew sign-on, task-by-task hazard and control listing, and PPE verification.

Spill & Release Report

Environmental release record: substance and estimated volume, source and cause, containment and recovery actions, receptors affected, and the notification log regulators will ask for.

Hot Work Permit

Pre-work authorization for welding, cutting, grinding and open flame: area survey, combustible controls, gas testing, fire watch assignment, and the post-work monitoring sign-off.

Confined Space Entry Permit

Permit-required confined space entry: hazard identification, isolation and lockout, atmospheric testing before and during entry, ventilation, rescue arrangements, and the entrant and attendant log.

Lockout / Tagout Verification

Energy isolation record: every energy source identified and controlled, locks and tags applied by name, zero-energy verification by test, and the sequenced release back to service.

Incident & Injury Report

First report of an injury, illness, property damage or vehicle incident: what happened in sequence, who was involved, treatment given, immediate causes and corrective actions, with witness statements and scene photos.

Near Miss Report

Short, low-friction report for the incident that almost happened: what occurred, what could have happened, the hazard behind it, and the fix — designed to be filled in under two minutes so it actually gets filled in.

Safety Observation Card

Behaviour-based observation card recording both safe and unsafe acts and conditions, whether the worker was spoken with, and the agreed correction — the leading indicator that shows up before incidents do.
